What is OCD?
Obsessive-compulsive disorder (OCD) is a mental health condition characterized by obsessions, compulsions, or both. OCD is more than simply being anxious, particular, or worried. It involves a cycle in which unwanted thoughts, images, urges, sensations, or doubts become difficult to dismiss, leading a person to perform behaviors or mental rituals to reduce distress or gain a sense of certainty.
What are obsessions?
Obsessions are unwanted, intrusive thoughts, images, urges, sensations, or doubts that repeatedly enter your mind and cause distress. They often focus on something that feels especially important, threatening, or inconsistent with your values.
OCD obsessions can take many forms. Someone might worry that they could accidentally hurt someone, question whether they truly love their partner, fear becoming contaminated, worry about making a mistake, experience disturbing sexual or religious thoughts, become preoccupied with bodily sensations, or repeatedly question whether they are making the "right" decision.
Importantly, having an intrusive thought does not mean you want it, believe it, or will act on it. Many people with OCD become distressed precisely because the thought feels so inconsistent with who they are or what matters to them.
What are compulsions?
Compulsions are behaviors or mental acts that you feel driven to perform in response to an obsession, intrusive thought, or feeling of uncertainty. The goal of a compulsion is often to prevent something bad from happening, reduce anxiety, or achieve a feeling of certainty or "just rightness."
Some compulsions are easy to recognize, such as:
Checking locks, appliances, or messages repeatedly
Washing or cleaning excessively
Repeating actions until they feel right
Asking others for reassurance
Avoiding people, places, or situations that trigger OCD
However, many compulsions happen entirely in your mind. These can include mentally reviewing memories, analyzing your feelings, comparing possibilities, trying to prove that a thought isn't true, mentally repeating phrases, replacing an unwanted thought with a "good" thought, or repeatedly asking yourself questions in an attempt to reach certainty.
These are sometimes called mental compulsions or covert compulsions.
The OCD cycle
OCD often operates as a cycle:
Intrusive thought, image, urge, sensation, or doubt → anxiety or uncertainty → compulsion or avoidance → temporary relief → more OCD
A compulsion may provide relief in the moment, but that relief teaches the brain that the thought or uncertainty was something that needed to be resolved. Over time, this can make the obsessions feel more urgent and the compulsions more difficult to resist.
This is why trying to get complete certainty about an OCD fear can actually keep the OCD cycle going.
OCD isn't always about visible rituals
One of the biggest misconceptions about OCD is that it always involves obvious behaviors like excessive cleaning or checking. OCD can also involve intrusive thoughts, mental compulsions, reassurance seeking, avoidance, rumination, and attempts to control or eliminate unwanted thoughts.
Some people with OCD may even appear to have no compulsions at all. This is sometimes referred to as "Pure O" or primarily obsessional OCD, although the term can be misleading because people with this presentation often have less visible mental compulsions or avoidance.
OCD can also take many different forms, including contamination OCD, harm OCD, relationship OCD (ROCD), health OCD, sexual OCD, religious or scrupulosity OCD, existential OCD, perfectionism-related OCD, sensorimotor OCD, and other forms of intrusive-thought OCD.
The specific content of an obsession is less important than the cycle of obsession, uncertainty, and compulsive responding that maintains it.
How is OCD treated?
OCD is highly treatable. Exposure and Response Prevention (ERP) is a specialized form of cognitive behavioral therapy and is considered a first-line psychological treatment for OCD.
Rather than trying to eliminate intrusive thoughts or prove that your fears are impossible, ERP helps you gradually approach uncertainty and feared experiences while learning to respond without compulsions, reassurance, avoidance, or other behaviors that keep the OCD cycle going.
At Still Minds Psychology, we specialize in OCD therapy using ERP, with treatment tailored to the specific symptoms, compulsions, and goals of each client.
If you think you may have OCD, you don't need to be certain about your diagnosis before reaching out. An OCD specialist can help you understand what you're experiencing and determine whether specialized OCD treatment is appropriate.
OCD Symptoms & Presentations We Treat
OCD doesn't always look like repetitive handwashing or checking. Obsessions and compulsions can take many forms, and some compulsions are almost entirely mental.
Contamination OCD-A fear of germs or dirt, leading to excessive washing, cleaning, or avoiding certain objects or places.
Checking OCD-Constantly checking locks, stoves, or light switches, driven by fear that something bad will happen if they are not checked repeatedly.
Harm OCD-Disturbing, unwanted thoughts or urges, such as fears of harming others, harm coming to animals, images of acting violently, thoughts of harming yourself.
Relationship OCD (ROCD)- Fear about our relationship such as the belief our partner is not right for use that cause immense anxiety and often result in repetitive reassurance seeking, rumination, and avoidance.
Health OCD- a fear of having or developing serious health issues. This often shows up a frequent body checking, medical avoidance or excessively going to the doctor, researching, and reassurance seeking
Sexual OCD- obsessions are about sexual questions or images. These often taboo and ego dystonic thoughts center sexual images, incestious thoughts, bestiality thoughts, and more. This can lead to extreme compulsive avoidance, rumination, and testing your sexuality. Other related obsessions include questioning your sexual orientation or gender identity
Religious or scrupulosity OCD- intrusive thoughts and compulsive behaviors related to religious or moral issues, Compulsive behaviors aimed at preventing sin or ensuring moral purity, such as excessive praying or confessing , checking and rechecking actions for mistakes
Moral OCD- concerns about making moral “mistakes” that might make them a terrible person. This often leads to rumination
Perfectionism-related OCD- feeling that every process and space needs to be perfected is a common form of OCD. Compulsions are often physical involving trying to make spaces perfect & organized, but mentally there can be compulsive strategizing trying to maximize for efficiency
Pedophilia-themed OCD (POCD)-repeated fearful thoughts that we may be or become attracted to children often resulting in avoiding content relating to children, checking arousal, seeking reassurance from others, even avoiding some common parenting tasks such as bath time and diaper changes. This can often effect parents and those who work with children.
Sensorimotor OCD- a form of OCD where the obsession is around sensations in the body. Oftentimes things like touch, breathing, blinking ect. not feeling “right”, and feeling a need to take control of automatic bodily fubnctions
Existential OCD- a preoccupation with existential questions like “what happens after we die”, “how can I be sure of my reality” which frequently leads to compulsive rumination and researching
Pure O / primarily obsessional OCD- You may notice you’re stuck in cycles of rumination, trying to mentally distract away from intrusive thoughts. While the types of thoughts can vary you will likely be struggling with many “what ifs”
Intrusive thoughts
Mental compulsions
Reassurance seeking
Exposure and Response Prevention (ERP) for OCD
Exposure and Response Prevention (ERP) is a specialized form of cognitive behavioral therapy and one of the most effective evidence-based treatments for OCD. ERP helps you gradually face situations, thoughts, sensations, or uncertainties that trigger OCD while learning to resist compulsions and other behaviors that keep the OCD cycle going.
Rather than trying to eliminate intrusive thoughts or achieve complete certainty, ERP teaches you that you can experience uncertainty and discomfort without responding to OCD. Over time, this can reduce the power OCD has over your choices and daily life.
ERP can be used to treat many forms of OCD, including intrusive thoughts, Pure O, relationship OCD, harm OCD, contamination OCD, health OCD, sexual OCD, religious or scrupulosity OCD, sensorimotor OCD, and other OCD presentations.
At Still Minds Psychology, ERP is individualized to your specific OCD symptoms, compulsions, and goals. Treatment may involve in-session exposures as well as practicing response prevention between sessions.

Obsessive–Compulsive Disorder (OCD) is a mental health condition driven by intrusive thoughts and the intense urge to reduce the anxiety they cause. These thoughts are unwanted, distressing, and often feel completely out of character—yet they can be incredibly convincing.
At Still Minds Psychology, we often see OCD misunderstood as being about cleanliness or organization. In reality, OCD can take many forms, and much of it happens inside the mind.
OCD typically involves:
Obsessions: intrusive thoughts, images, urges, or doubts that cause distress
Compulsions: behaviors or mental acts done to reduce anxiety, seek certainty, or “undo” the thought
Relief from compulsions is usually temporary, which is how OCD keeps the cycle going.
Signs You May Be Experiencing OCD
You might be dealing with OCD if you notice patterns like:
Intrusive thoughts that feel disturbing, taboo, or opposite of your values
Repetitive mental checking, analyzing, or “figuring it out”
Strong urges to seek reassurance from others or online
Avoiding people, places, or situations to prevent certain thoughts
Feeling responsible for preventing harm, even when it’s unrealistic
Difficulty tolerating uncertainty or doubt
Thoughts that feel urgent, sticky, or impossible to let go of
Spending hours each day caught in mental loops, rituals, or avoidance
Many people with OCD worry that having these thoughts means something about who they are. It doesn’t. OCD targets what you care about most—and uses fear to demand certainty.
The Good News
OCD is highly treatable with the right approach. Therapy focuses on changing how you respond to intrusive thoughts—not getting rid of them entirely or proving they “don’t mean anything.”
You don’t need to be certain this is OCD to reach out.
You don’t need to explain everything perfectly.
And you don’t need to keep managing this alone.→ Learn how therapy for OCD works

If you’re living with OCD or anxiety, you may already worry that therapy could make things worse. It’s important to be clear about what you won’t experience at Still Minds Psychology.
ERP is not about increasing intrusive thoughts on purpose or flooding you with fear.
ERP does not aim to shock or overwhelm you.
Intrusive thoughts may already fluctuate naturally; ERP does not create new ones or make them more “dangerous.”
What ERP does change is your relationship to the thoughts.
Many people notice that anxiety can rise temporarily at the start of ERP—not because thoughts are getting worse, but because the usual compulsions (reassurance, checking, avoidance) are no longer being used to get quick relief. Over time, as your brain learns that the thoughts don’t require action or certainty, their intensity and pull typically decrease, even if they still show up occasionally.
ERP here is:
Collaborative and paced
Tailored to your specific OCD theme
Adjusted if something feels too much or unhelpful
Focused on long-term relief, not short-term pushing
You will never be asked to do something without understanding why, and you always have a say in how therapy unfolds.
Therapy here is not:
Reassurance-seeking or constant certainty-checking
Judging, analyzing, or assigning meaning to your thoughts
Being told to “just stop thinking that way” or “let it go”
Forcing exposures before you feel ready
Confession-based or moral evaluations of your thoughts
A place where you have to explain, defend, or censor yourself
Intrusive thoughts can be graphic, disturbing, or completely opposite of your values. They do not shock us, and they do not define you.
Therapy here is about learning how anxiety and OCD operate, building skills that reduce their grip, and helping you respond to thoughts in ways that bring relief—without feeding the cycle. We work collaboratively, transparently, and at a pace that feels manageable.
-
While each person is unique in how OCD shows up, the research is clear that ERP is the gold standard treatment for OCD. At Still Minds Psychology we are highly trained in providing ERP. Other research backed OCD therapies include things like ACT and I-CBT. These are recommended second line treatments, meaning if you have tried ERP and have not gotten the symptom reduction you need, your therapist may incorporate these approaches to help you get the best result. Meghan often utilizes techniques from these other evidenced based therapies to help you get the best result.

Item Intrusive thoughts can be graphic, disturbing, or completely opposite of your values. They do not shock us, and they do not define you.
Intrusive thoughts can be shocking, graphic, or frightening precisely because they clash with who you are. Having these thoughts does not mean you want them, believe them, or are capable of acting on them.
Therapy here is about helping you understand why these thoughts happen, how OCD uses fear and doubt to stay powerful, and how to respond in ways that reduce shame, anxiety, and mental checking—without feeding the cycle. We move carefully, collaboratively, and with respect for your values and boundaries.
You will not be punished for your thoughts here.
You will not be misunderstood for having them.
You are allowed to go slowly.Meghan has worked with pretty much every OCD theme including POCD, sexually themed OCD, harm OCD, incest OCD, and more

That’s a completely valid concern, and one we hear often. Many of our clients have been through therapy before and felt like they only ever learned surface level coping skills or were just rehashing their obsessions over and over again, only getting reassurance that provided temporary relief.
At Still Minds Psychology, our team is trained specifically in treating OCD. We use evidence-based methods like ERP & ACT, strategies that go beyond surface-level coping. Therapy with us is structured, personalized and rooted in approaches proven to help people reduce OCD and build meaningful, lasting change.
-
That’s extremely common. We’ll go at a pace that feels manageable and respectful. While treatment may cause anxiety, that does not mean it isn’t healthy, or the right direction. Sometimes growth can be uncomfortable.
Besides, even without treatment just living with OCD is guaranteed anxiety

Yes, ERP is the #1 evidenced backed therapy designed to work with intrusive thoughts. It’s a common misunderstanding that ERP can only work with people who have physical compulsions, because research shows ERP is the most helpful regardless of which type of intrusive thought or compulsion you are struggling with.

Yes, ERP is the #1 evidenced backed therapy designed to work with OCD including Pure-O. It’s a common misunderstanding that ERP can only work with people who have physical compulsions, because research shows ERP is the most helpful regardless of which type of intrusive thought or compulsion you are struggling with. ERP addresses the underlying patterns of OCD, not the surface compulsion. At Still Minds Psychology we are highly experienced helping those with Pure-O recover, and will also incorporate other evidenced backed approached like ACT and I-CBT to aid in client’s recovery.

Yes! We now have substantial research to show ERP and OCD treatment delivered via telehealth is equally effective to in person treatment. Telehealth actually creates a unique opportunity for OCD treatment, as doing exposures for OCD is often best done in your home/real life environment. We often do exposures with people while at home, work, or wherever else they may experience OCD triggers. This allows you to get the best treatment possible without having to pay the cost for a therapist to travel to exposure sites, making treatment more accessible for many.

Having intrusive thoughts does not mean you’ve done anything wrong, and it does not automatically require reporting.
Intrusive thoughts are incredibly common. Many people experience unwanted thoughts about harm, sexuality, aggression, or other disturbing themes. These thoughts can feel upsetting or even frightening , but a thought is not the same as an action or intent. In therapy, you can talk openly about intrusive thoughts without fear of judgment.
As a licensed therapist, I am required to break confidentiality only in very specific situations , such as if there is an immediate and serious risk of harm to yourself or someone else, or in cases involving abuse of a child, elder, or dependent adult. Simply having intrusive thoughts does not meet that threshold.
Therapy is a space to explore what’s happening in your mind safely and honestly. You do not have to carry scary or unwanted thoughts alone.
